Who is Eligible?*
Families with children from the ages of 3-5yrs old.
Families with income at or below the federal poverty guidelines.

What should I bring?
1. Child's Birth Certificate
2. Child's latest Physical & Immunization Record with Lead Screening
3. Child's Social Security Card.
4. Child's Medical Card.
5. Proof of address and proof of income
6. Copy of IEP(if child has one)

Please be advised that due to the limited time and space we will only be able to service the first 40 families that sign in.
