Job Title:                     Adoption Support and Preservation Worker
                                   
Reports to:                  Manager of Permanency Services
 
Bachelors degree in Social Work or other related field from an accredited school.  Must have a good working knowledge of clinical social work services with adopted children and their families.  Must have crisis intervention experience.  Must have a valid driver’s license, and car, and auto liability insurance.

Job Responsibilities

  1. Provide comprehensive case management services to families involved in the Adoption Support and Preservation Program.  These services may include any of the following: Advocacy, guided referral, parenting education, support, information, and case coordination.  These services may be provided in the client’s home, in the office, or in the community.
  2. Assist the program clinician in assessing family needs and developing a service plan.
  3. Assist the program clinician in providing family and group intervention.  This includes co-facilitating Parent’s Support and Education Groups and children’s groups.
  4. Actively collaborate with team members and other related service providers to ensure the provision of high quality services to program clients.
  5. Contribute to the overall success and quality of the program by working constructively and collaboratively with other staff members to address programmatic and agency-wide issues and needs.
  6. Collaborate with other service systems including; DCYF, legal, judicial, medical, educational, human services, etc., on behalf of program clients.
  7. Maintain client records and complete all necessary documentation within established program time frames.
  8. Maintain current information regarding agency programs, along with information regarding insurance eligibility and other community resources. This includes assisting in publicizing agency programs and educating others about the agency and its services.
  9. Constructively utilize formal and peer supervision to evaluate and improve effectiveness in providing services to clients.  This includes active participation in individual supervision, team meetings, child welfare team meetings and agency meetings.
  10. Participate in the development and implementation of training programs for professionals per contract requirements.
  11. Participate, as required, with administrative staff in the evaluation of agency programs and service delivery.
  12. Participate in community networking activities based on program or agency need.  Participate in staff committees as assigned.
  13. Pursue ongoing professional development through professional reading and attending relevant workshops and conferences (including Children’s Friend’s core training program).
  14. Interact effectively and respectfully with others and particularly with individuals whose backgrounds and life experiences are different from one’s own.
  15. Maintain a caseload as established by the Program Manager.
  16. Assist in publicizing agency programs and educating others about the agency and its services.
  17. Meets productivity standards for the program.
  18. Complete other duties as assigned.
