Job Description
Job Title: Clinical Social Worker, Nurse-Family Partnership
Reports to: Nurse Supervisor, Nurse-Family Partnership
Qualifications: Masters Degree in Social Work or other Mental Health field from an accredited school. LICSW or equivalent required. Expertise in mental health services for vulnerable, young children and families required. Must have a valid driver’s license, and car, and auto liability insurance
Duties
- Provide consultation and support to staff within the Nurse-Family Partnership program using the principles of reflective practice .
- Collaborate with staff in developing service plans for each family. Specify problems and goals related to mental health.
- Participate in case conferences, team meetings and home visits with program staff as needed.
- Constructively utilize individual and peer supervision to evaluate and improve effectiveness in providing services to clients. This includes active participation in reflective supervision, team meetings and agency meetings.
- Contribute to effective leadership of Children’s Friend through a commitment to excellence, modeling for others and building a culture of success.
- Collaborate and communicate with team members and other Children’s Friend staff to ensure appropriate and effective services for families. Make referrals for services as needed.
- Actively participate on the community task forces and committees as assigned. Demonstrate professionalism, competence and expertise when representing the agency in the community.
- Contribute to the overall success and quality of the program by working constructively and
collaboratively with other staff members to address programmatic and agency-wide issues and needs.
- Pursue ongoing professional development through professional reading and attending relevant workshops and conferences (including Children’s Friend’s core training program)
- Ensure that all individuals are treated respectfully and that diversity in backgrounds and life experience is supported and valued throughout the agency.
