Job Title: FCCP Intake Worker (Temporary—6 Months)

Reports to: Supervisor/Wraparound Coach

Qualifications: At least an Associates Degree in Social Work, Child Development or a related field with one year’s experience working with at-risk families.  Bachelor’s Degree preferred. Bi-lingual (English/Spanish) preferred.  Must have and maintain: a valid driver’s license; an automobile fit to be used for program related purposes; and, automobile insurance in an amount acceptable to Children’s Friend.

Able to work constructively with clients, agency staff, and other providers.  Able to follow directions from supervisors and managers.  Able to communicate clearly to clients, agency staff, and other providers, in all forms of media  the agency’s and program’s goals, details, and resources.

Duties

Processes and screens applicants and referrals to the FCCP including documenting all referral information, following-up with clients or other parties to obtain needed and/or missing information, and communicating to clients the status of the referral.

Assess the clients’ suitability for wraparound including orienting the client and/or referral source, and supporting the client by home visits or via telephone.

Timely document and record all program and agency required information.

Link clients to resources, supports, services that are appropriate to the clients’ needs.

Will perform home visits as assigned..

Other duties as assigned.
