Title:                                      Project Connect Worker (Temporary—6 Month Position)
                                   
Reports To:                            Project Connect Manager and/or Project Connect Supervisor

Supervises:                             N/A

Minimum Qualifications:        Master’s Degree in Social Work or a related field and at least 2 years experience with at-risk families, substance abuse, and service coordination.  A Bachelor’s Degree in Social Work or related field with at least 3 years experience with at-risk families, substance abuse, and service coordination may be substituted.  An appropriate professional license and/or bi-lingual (English/Spanish) preferred. Must have and maintain a valid driver’s license and auto liability insurance in an amount acceptable to Children’s Friend.

Abilities:    Able to work constructively with clients, agency staff, and other providers.  Able to follow directions from supervisors and managers.

Duties and Responsibilities

Receives and reviews cases from DCYF workers; assess each referral per program standards and guidelines and assign cases to staff as required.

Develops and implements a service plan for each family per the program’s guidelines and standards.

Assesses client’s substance abuse problems; works with clients to develop an active recovery plan and links clients to appropriate services.

Maintains contacts with all other service providers and/or involved entities. Coordinates case conferences on a clinically appropriate schedule.

Prepares all needed reports, including progress reports, termination summaries, and insures that all documentation is accurate and up-to-date per the program’s guidelines and the agency’s policies.  Distributes reports and documentation to all appropriate parties.

Pursues ongoing professional development through professional reading and attending relevant workshops and conferences (including Children’s Friend’s core training program).

Assists in publicizing program/other agency program and educating others about the agency and its services.

Consistently meets productivity requirements of the program and the agency including at least 21 contact hours per week.

Other duties as assigned.
