Family Outreach Program
Information on at-risk children is sent to Children’s Friend on daily basis and we arrange a home visit with families interested in receiving additional supports and information. Ideally these visits take place within a few days of a mother’s return home from the hospital. The goal of this program is to respond to “high risk” families within 24 hours and to visit all families within 7 days of the referral. Our bi-lingual team of nurses and social workers is knowledgeable about infant growth and development, breast-feeding, postpartum depression and we serve as a referral source for the new parent within the community. Children’s Friend provides services under the Family Outreach Program in Central Falls, Pawtucket, East Providence and parts of Providence. The Family Outreach Program is a home-based outreach service intended to promote the health and well-being of mothers and their newborn children. Mothers and newborns, with potential risks for healthy development, are identified by the hospital when the baby is born. Eligibility criteria are different for every family, depending on the age of the mother, health history, pre-maturity, and involvement with the Department of Children Youth and Families. In addition to seeing infants, Children’s Friend reaches out to young children who are entering the foster care system.
