Job Description

Title: Nurse – Family Outreach Program

Reports To: Health Services Manager

Qualifications: RN with current Rhode Island license.  Must have experience working with pregnant women and expectant families, young children, and at-risk families.  Must have a valid driver's license, auto liability insurance, and a car to be used for work related tasks.

Duties:

  1. Provide home-based family assessments of families participating in the Family Outreach Program, using the Family Assessment Tool, capturing health, development, psychosocial, and environmental risks.
  2. Provide Infant Assessment Screening or Ages and Stages Questionnaire Screening on all children participating in the Family Outreach Program.
  3. Coordinate with local and state community resources regarding medical home, immunizations, and lead screenings.
  4. Assist with connecting eligible families with area WIC programs.  Identify local health resources and eligibility requirements for accessing those resources.
  5. Ensure that all families of babies with significant metabolic or hemoglobinopathy test results receive necessary follow-up care.
  6. In coordination with FOP Social Worker and Health Services Manager, ensure that all families are receiving parenting education.
  7. Maintain client records and complete all necessary documentation within established program time frames.
  8. Adhere to productivity requirements as established for the Family Outreach Program.
  9. Contribute to the overall success and quality of all programs and services within Children’s Friend through working constructively and collaboratively with other staff members to address programmatic needs and agency-wide issues and needs.  Be willing to assist other programs and services with health related needs.
  10. Constructively utilize formal and peer supervision to evaluate and improve effectiveness in providing services to clients.  This includes active participation in supervision, team meetings and agency meetings.
  11. Pursue ongoing professional development through professional reading and attending relevant workshops and conferences (including Children’s Friend’s core training program).
  12. Develop and maintain ongoing outreach strategies to inform families and the community of agency programs, services, and activities offered through Children’s Friend.
  13. Interact effectively and respectfully with others and particularly with individuals whose backgrounds and life experiences are different from one’s own.
  14. Other duties as assigned.

Job Description

Title: Nurse Family Support Services

Reports To: Health Services Manager

Qualifications: RN with current Rhode Island license.  Must have experience working with pregnant women and expectant families, young children, and at-risk families.  Must have a valid driver's license, auto liability insurance, and a car to be used for work related tasks.

Duties:

  1. Provide home-based health assessments of pregnant women, young children and families as needed, and share information with appropriate program staff.
  2. Provide consultation to program staff regarding child and parent health needs.
  3. Track health information on program participants, and prepare required reports.
  4. Maintain client records and complete all necessary documentation within established program time frames.
  5. Develop and implement wellness education programs, including smoking cessation and other topics of interest to program participants and the community.
  6. Identify local health resources and eligibility requirements for accessing those resources.
  7. In coordination with the Health Services Manager, Education Manager and Family Services Managers, ensure that all children with special needs are receiving appropriate services.
  8. Assist in creating opportunities for meaningful parent involvement.
  9. Meets established productivity standards for the position.
  10. Constructively utilize formal and peer supervision to evaluate and improve effectiveness in providing services to clients.  This includes active participation in supervision, team meetings and agency meetings.
  11. Pursue ongoing professional development through professional reading and attending relevant workshops and conferences (including Children’s Friend’s core training program).

Job Description

Title:  Early Intervention Qualified Personnel (Occupational Therapist, Nurse, Clinical Social Worker, Physical Therapist, Dietician, Speech and Language Pathologist, Early Childhood Educator)                 

Reports to: Early Intervention Program Manager/Clinical Supervisor

Qualifications:  Highest degree applicable to the profession/discipline such as Masters in Social Work, Masters in Speech and Language Therapy, BA in Early Childhood Education or Special Education, BS in Nursing, BS in Physical Therapy, BS in Occupational Therapy and appropriate state licensure. Must have 1-2 years experience working with young children and families, including knowledge of or experience with disabilities. Must have current license as required by individual’s profession. Must have driver’s license, automobile, and appropriate insurance levels.

Duties:

  1. Implement Early Intervention services that conform with all federal, state and agency standards.
  2. Participate in program procedures including intake, assignment of cases, and the utilization of a multidisciplinary team for evaluations and program services.
  3. Coordinate service delivery including those with outside vendors and practitioners.
  4. Maintain client records and complete all necessary documentation within established program time frames.
  5. Within six (6) months of employment, complete Introduction to Early Intervention course.
  6. Complete a minimum of twelve hours per year of training with a focus on working with young children with disabilities and their families.
  7. Attend and participate in all relevant state early intervention meetings.
  8. Constructively utilize formal and peer supervision to evaluate and improve effectiveness in providing services to clients.  This includes active participation in supervision, team meetings and agency meetings.
  9. Pursue ongoing professional development through professional reading and attending relevant workshops and conferences  (including Children’s Friend’s core training program).
  10. Ensure appropriate practices and systems of Early Intervention billing, in collaboration with the Finance Department.
  11. Ensure cross program/ agency collaboration and communication to facilitate coordinated services for early intervention children and families.
  12. Contribute to the overall success and quality of the program through working constructively and collaboratively with other staff members to address specific programmatic needs as well as agency wide issues and needs.
  13. Assist in publicizing agency programs and educating the community about early intervention services as well as other agency programs.
  14. Meets established productivity standards for direct service.
  15. Interact effectively and respectfully with others and particularly with individuals whose backgrounds and life experiences are different from one’s own.
  16. Other duties as assigned.
