Receive referred cases from Project Connect workers. Provide home-based parent assessment, education and supportive services to referred families. Conduct necessary assessments to determine areas where parent need support and education. Supervise visits as needed. Collaborate with the team on recommendations.
Maintain client records and complete all necessary documentation within established program time frames. Participate in the family service planning process.
Interact effectively and respectfully with others and particularly with individuals whose backgrounds and life experiences are different from one’s own.
